Question from Christy, Mar. 30, 1:58:18 AM:   My vet has suggested using gabapentin for my 90 lb 11 yr old lab for pain management of his severe arthritis.  Is this a good thing?

Hi Christy.  I love gabapentin. It works really well. It can take 2-3 weeks to work and is a longer term medicine, but provides good pain relief. I use a dose of about 10-15mg/kg once daily.

Question from Michelle, Mar. 29, 12:25:43 AM:   I am just starting my 16 year old dog on Gabapentin. He already takes Deramaxx in the AM and Tramadol in the PM. Can I give him the Deramaxx and Gabapentin at approximately the same time?

Hi Michelle! Yes, you can give gabapentin and tramadol at the same time. Often Tramadol is 2-3 times a day anyway and gabapentin can be twice daily as needed. Supplements can also help Susan and I have some great suggestions here, so let me know if you would like to talk about them.

Question from Marco, March 29, 12:20:38 AM:   I have a 15 month old Boerboel (South African Mastiff). She has hip dysplasia on both sides. My vet wants to do the operation that removes the femoral head. They tell me that to do a total hip replacement would cost $10K. I see mixed remarks concerning the femoral head removal. She is 90 pounds now but will easily reach 120 when completely grown. Is the femoral head removal okay for a dog this size?

Hi Marco, is your dog lame at the moment? By her age (as in she has reached maturity) a lot of dogs are not sore at all. I would not be in a rush for any surgery if she is not sore. Let me know and we can keep chatting via email.

Hi Dr. Chris!

I have a 7 yr old pit bull mix whose hind quarters are a mess.  He has had dysplasia in the left hip that is actually more subluxated… ( I think that’s what they said–the joint is not at all connected) His left femur is visibly shorter than the right.   He is a rescue so I always wonder if he got injured- or if he was born this way.  Anyway, all this mess on the left side causes him to primarily use his right rear leg, which is visibly bigger.   He blew that knee out couple of years ago and had TPLO surgery.  He has been doing fine but I one day recently he was very reluctant to walk or move.  I had him x-rayed and they say the TPLO knee has arthritis and also he has some arthritis in the  base of his spine.  None in the left hip since nothing is touching.

Anyway long story…I am wondering what kind of things I can do to avoid drugs.  What I am really curious about is can I help make that left leg more useful? I am interested in massage too.  He is not overweight.  He has been on Dasuquin forever, and I recently started giving him Omegas, and wondering about eggshell membrane and green lip mussels.

Hi Sylvia, thanks for contacting me.

It sounds like a real challenge for you and your boy :(

I guess you need to work out why he is so lame in the left leg. A subluxated hip will often be pain free but not always. Hip, back or knee pain are possible and your vet should be able to pick the difference. Treatments vary from there of course. Muscles can also be super sore.

It sounds like your supplements are ok but if you are trying to avoid prescription drugs you may need to ramp them up. Not sure about egg shell membrane but the basics you have in place. My ‘Modern Dog Arthritis Treatment’ book can walk you through the minefield of supplements quite nicely and with the flexibility you will likely need. Is $9, I can send you a link if you are interested.

I think you can make the left leg more useful. Massage, physical therapy etc. can all help and may allow him to be off the prescription drugs… but I think you need to localise the pain and clarify the cause of the lameness before you really push down this path. You can waste a lot of time if the diagnosis is not accurate. I would lean on your vet for a more clear answer.

Lastly, don’t turn your back completely on the prescription drugs. They are mostly well tolerated and can work much faster than the supplements. Drugs like gabapentin for back pain are an absolute god send for sore dogs. Even the NSAIDs can make a huge difference for the right patient.
